A four-hundred-mile journey is a different proposition from an airport run. We plan driver hours and breaks properly rather than hoping for the best.

For one person travelling with an advance ticket, usually not. For three or four people, or anyone with heavy luggage, awkward timings or a destination away from a station, it very often is once you add the taxis at both ends.
On distance, vehicle size and timing, quoted as one fixed figure before you commit. It includes fuel, tolls and the driver's time, and it does not change if the traffic does.
Long-distance pricing accounts for the vehicle returning, so a one-way journey costs more per mile than a return. Booking both legs together is usually noticeably better value.
Yes on the great majority of journeys. On very long runs we may assign two drivers for safety, and we will tell you that when we quote.
Routinely. The office is staffed 24 hours and overnight runs often make sense, avoiding motorway congestion entirely.
